Requesting Certificates (for Department Graduates)

Information on the procedure for graduates requesting certificates is indicated below.
Please send requests via post or in person. As a general rule, requests will not be accepted via fax, telephone, or e-mail.

  • Certificates are issued in the name of the student at the time of enrollment.
  • Graduates of postgraduate courses should contact the Office of the Graduate School directly (daigakui@kufs.ac.jp).

  • As a general rule, letters of recommendation are created by full-time faculty members at KUFS. Please select and contact the faculty member to request the letter of recommendation. Make sure to obtain agreement and convey the address to send the letter to as well as any deadline, etc. If you cannot contact the faculty member to make the request to, please contact the Division of Academic Affairs. If a predetermined format is required, send it together with the recommendation letter request, after filling out the applicant information in advance (such as the course you have applied for, address, name, and graduation thesis, etc.). The letter will arrive in a sealed envelope, so that anyone opening it will know it has not been tampered with or altered in any way, so please note that you as the applicant should not open this letter. If a predetermined format is not required, please state this clearly in your request for a letter of recommendation.

Billing procedure

Please send the following three items to the Certificate Issuance Section at the university.

  1. Certificate Issuance Request Form (PDF file) which can be found at the end of this page
  2. Copy of passport (photo page)
    • If your name at time of enrollment differs from your current name, please submit documents from a public institution that proves the association between the two names.
  3. Postage stamps (total amount including the issuance fee and return postage fee)
    • For applicants in Japan, the return postage fee may be substituted with a Letter Pack, a postage-prepaid envelope.
    • Please note that excess stamps are non-refundable.
    • Please do not enclose cash, postal money orders, or revenue stamps.
    • When making an application from overseas via post, International Reply Coupons can be used instead.
    • Letter Packs can be purchased at post offices or convenience stores.

    [Regarding International Reply Coupons]

    • International Reply Coupons are stamps that can be purchased at relatively large post offices in countries overseas.
    • Please send an amount equivalent in Japanese yen. (One International Reply Coupon is worth 130 yen in Japan.)
    • Please send coupons that are valid for at least three months.

<Applications via proxy>

Please follow the procedure below.

  1. Applicant (graduate): Send the following three items to a proxy (residing in Japan) via e-mail.
    1. Certificate Issuance Request Form (PDF file)
    2. Copy of passport (photo page) (PDF file)
    3. "Letter of Proxy" signed by the applicant (PDF file)
  2. The proxy must send the following five items to the Certificate Issuance Section at the university.
    1. Request for Issuing Certificate in 1. - (1)
    2. Passport in 1. - (2)
    3. Letter of Proxy in 1. - (3)
    4. Copy of personal identification of proxy
      • Driving license, Health Insurance Card, My Number Card, Residence Card, etc.
    5. Postage stamps (total amount including the issuance fee and return postage fee)
      • Return postage fee may be substituted with a Letter Pack, a postage-prepaid envelope.
      • Please note that excess stamps are non-refundable.
      • Please do not enclose cash, postal money orders, or revenue stamps.
      • Letter Packs can be purchased at post offices or convenience stores.

  • On the address on the lower right of the Certificate Issuance Request Form, please circle either Mr. or Ms., write your address in English, and be sure to include your postal code and telephone number.
  • Currently, shipping to Ukraine, Belarus, and Russia is temporarily suspended.
